Lifeline Home Care has expanded their services and offer to their clients by opening its new division, Extended Care. Lifeline Extended Care features home-style Assisted Living Facilities designed to allow their clientele the opportunity to enjoy full time CNA and nursing assistance, while living in a small group home setting.

The facility is located in historic Warrenton, Fauquier County, in a tranquil setting with spacious living accommodations, tastefully decorated and furnished with memorabilia belonging to their residents.

Services are fully comprehensive and geared towards the individual needs of their clients. A case manager will evaluate the needs of each resident and a care plan will be established with the input of residents, family members, doctors and our own Nursing staff.

• 24 hour Residential accommodation
• Case Management
• CNA, PCA, LPN, & RN services & Nursing Assessments
• Nutritional Planning
• Three cooked meals daily, plus snacks
• Medication Management to include filling prescription, Med. set up & dispensing
• ADLs
• Incontinence supplies, gloves etc
• Transportation to and from Physicians’ appointments"
